Practice Information

We provide medical, dental, and behavioral health services across our locations. We apply clinical decision supports following evidence based guidelines. • Hours: Monday-Thursday 8a-5p; Friday 8a-3p
• No On-Call or Weekends
• Patient Volume: 12-14 per day plus additional Hygiene Checks; Will see patients of all ages
• Perform dental procedures such as fillings, extractions, simple root canals/crowns, restorations, dentures, and cleanings
• Diagnose and treat oral diseases and injuries
• Educate patients on oral health care and preventive measures
• Create treatment plans to maintain or restore patients' oral health Manage patient records and maintain confidentiality
• Collaborate with dental assistants and hygienists to provide comprehensive dental services

Financial Package

• Salary range: $170,000-220,000/year
• Sign on Bonus: Up to $20,000
• Health, Vision, Dental- United Healthcare paid at 95% for the employee with 3 different plans to select. Delta Dental Plan paid at 95% for the employee - 3 different plans to select
• Basic Life Insurance/ AD&D 1x annual salary paid by health center. Additional options for employees/family members
• Malpractice Insurance covered through Federal Torts Claims Act with lifetime tail
• ST/LT Disability paid for by health center.
• PTO: 160 hours annually
• $3,000 and 40 hours off for CME ; will cover licensure costs
• 8 Paid holidays
• 5%- 401k match
• PSLF Loan Repayment - National Health Service Corp/State Student Loan repayment, both $50,000 for 2 year commitment. Can renew
